While risperidone is an excellent medication, it can cause an increase in prolactin levels. By blocking dopamine 2 receptors in the pituitary, it can cause elevations in prolactin. Excess prolactin can cause the production of breast milk in men and in women who are not pregnant or breastfeeding. In women, too much prolactin can also cause menstrual problems and infertility (the inability to get pregnant). In men, it can lead to lower sex drive, erectile dysfunction (ED), and gynecomastia.
However, this is also one of the most effective medications that we have for Autism Spectrum Disorder related irritability (and is FDA approved in children ages 5 and up), Schizophrenia, Other psychotic disorders, Acute mania/mixed mania, Bipolar maintenance, impulse control disorders, and behavioral disturbances in children and adolescents.
There are various ways of counteracting this side effect and making this medication safer to use. We recommend starting Vitamin B6 300mg two times per day with this medication. Zhuo “Front Psychiatry” double blinded study demonstrated this lowered prolactin by 68% (vs 37% when abilify used - this is another wy to counteract this side effect). Vitamin B6 is an effective prolactin inhibitor that is extremely cheap and safe.it exerts hypothalamic dopaminergic effect which causes a significant reduction in prolactin level.
Where to purchase: (keep in mind we recommend taking 300mg two times per day, as what was used in the study referenced above) - most Vitamin B6 is supplied in 100mg, which would require 3 tablets two times per day
1. https://a.co/d/7FI6P47 *recommend this one, 250mg tabs...ok to take 1 twice per day and this is close enough to the 300 mg to have likely the same impact
